Indications/Uses
Major depressive disorder episodes, panic disorder w/ or w/o agoraphobia, social anxiety disorder (social phobia), generalised anxiety disorder (GAD), obsessive-compulsive disorder (OCD).
Dosage/Direction for Use
Major depressive disorder Usual dose: 10 mg once daily, may be increased to max of 20 mg daily. Usually 2-4 wk are necessary to obtain antidepressant response. Panic disorder w/ or w/o agoraphobia Initially 5 mg for 1st wk before increasing the dose to 10 mg daily, max of 20 mg daily. Social anxiety disorder Usual dose: 10 mg once daily. Usually 2-4 wk are necessary to obtain symptom relief, may be decreased to 5 mg or increased to max of 20 mg daily. GAD Initially 10 mg once daily, may be increased to 20 mg daily. OCD Initially 10 mg once daily, may be increased to 20 mg daily. Mild or moderate hepatic function Initially 5 mg daily for 1st 2 wk of treatment, may be increased to 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ non-selective MAOIs, moclobemide.
Special Precautions
Paradoxical anxiety. Discontinue if seizure develops. Development of akathisia. Hyponatremia. Patients w/ history of mania/hypomania, suicide-related events; diabetes; cirrhosis; CHD. Serotonin syndrome. Concomitant use w/ oral anticoagulants, atypical antipsychotics & phenothiazines, most TCAs, ASA, NSAIDs, ticlopidine & dipyridamole; St John's wort. Close monitoring in patients w/ controlled epilepsy. Discontinuation symptoms due to abrupt discontinuation of therapy. Careful monitoring for suicidal symptoms. Electroconvulsive therapy. Pregnancy & lactation. Childn >18 yr. Elderly >65 yr.
Adverse Reactions
Nausea; decreased &/ increased appetite, increased wt; anxiety, restlessness, abnormal dreams, decreased libido, anorgasmia; insomnia, somnolence, dizziness, paraesthesia, tremor; sinusitis, yawning; diarrhoea, constipation, vomiting, dry mouth; increased sweating; arthralgia, myalgia; ejaculation disorder, impotence; fatigue, pyrexia.
Drug Interactions
Irreversible non-selective MAOIs, Risk of serotonin syndrome w/ moclobemide, selegiline. Linezolid. Serotonin syndrome w/ tramadol, sumatriptan. May lower seizure threshold w/ TCA, SSRIs, neuroleptics, phenothiazines, thioxanthenes, butyrophenone, mefloquine, bupropion, tramadol. Enhanced effects w/ lithium, tryptophan. Increased incidence of adverse reactions w/ St. John's wort. Altered anticoagulants effects w/ oral anticoagulants. Alcohol.
